Your private records: In the undertaking of my work I may keep various details about you, such as your name, address, telephone number, email address, GP and information about your general health and wellbeing. This is recorded only to ensure I provide you with the most appropriate therapy. This information is only accessed by myself to enhance my service to you or to contact you about your appointment.

How I store your information: I store your basic Identification using a password protected computerised system that only I have access to. It can only be accessed by my login details and files are password protected. I back-up all data to a secure drop-box which is also password protected.

Sharing information: None of your details or information shall ever be sold, given, marketed, or shared with marketing organisations by me. There may be times when certain information may need to be passed on to someone else details of this can be found in the full privacy policy.

Your right to access information: You have the right to request access to all the information I hold about you within 30 days of the request.

Your right to be forgotten: You have the right to request for your details to be deleted. This is not an absolute right because in healthcare legislation I am required to keep your details for longer. In general I keep client details for 8 years after the end of our last contact then they will be securely destroyed.

Clinical Supervision: I have regular supervision with more experienced colleagues in my psychotherapeutic work. In these sessions I am expected to discuss my work but I do not divulge personal client details.

Your consent: Under the General Data Protection Regulation (GDPR) I am required to ask for consent from you to keep your information. This form will be completed at the first consultation.
